Epitalon: what the studies show

Every line below comes from a study, label or regulator record, checked by two models, with the source linked. Information, not medical advice. For adults 18+.

What it is

  • Epitalon (also spelled epithalon or AEDG) is a synthetic four-amino-acid peptide (Ala-Glu-Asp-Gly) built from the amino-acid makeup of epithalamin, a crude extract from the pineal gland.[1][2]
  • Epitalon and epithalamin are not the same substance. Epithalamin is a crude bovine pineal extract. Epitalon is the synthetic four-amino-acid peptide. FDA treats them as different. Most older human 'anti-ageing' trials used epithalamin, not epitalon. The knowledge base tags those trials to a separate group, the Khavinson bioregulators, so they appear on this card only as related-molecule evidence. Epitalon itself was first reported in a pineal extract in 2017.[1][2][3][4]
  • The research is concentrated in one group. 108 of the 166 PubMed records tagged epitalon list Khavinson or Anisimov among the authors (some are passing mentions). The studies are largely from Russian institutes and often in Russian-language journals.[5][6][7][2]

Bottom line

  • For epitalon itself, the human data are one short retinitis pigmentosa report with no design detail, one brief report on urine melatonin, and lab work on cells from people. The longevity and mortality results people quote come from trials of epithalamin, the pineal extract, not epitalon. The animal work is mostly from one group. Telomere lengthening is shown in cultured cells, and FDA flags this as a possible cancer concern.[1][6][8][4]

What the human studies found

  • Epitalon itself, retinitis pigmentosa: a 2002 report says epitalon treatment of patients with degenerative retinal disease gave a 'positive clinical effect in 90% of the cases'. The abstract gives no patient number, comparator, dose or outcome scale. The knowledge base labels it an RCT, but the abstract shows no sign of randomisation.[6]
  • Epitalon itself, melatonin: a 2020 Russian-language abstract says the AEDG peptide raised urinary 6-sulfatoxymelatonin 1.7-fold in middle-aged people and shifted clock-gene expression in blood cells of people with low pineal melatonin function (night workers). The abstract gives no group sizes, dose, control group or design.[8]
  • Epitalon itself, cells from people: AEDG-treated blood lymphocytes from 11 men showed significant telomere changes in 7, with five increases and two decreases. In another ex vivo study, epitalon and other short peptides changed chromatin markers in blood cells from people aged 75-88. This is cell culture, not treatment of people.[9][10]
  • Different molecule (epithalamin, the pineal extract, not epitalon): in a randomised comparison, 39 coronary patients got 6 courses of epithalamin over 3 years plus standard care and 40 controls got standard care alone. The authors reported slower cardiovascular ageing and lower mortality in the treated group. It is a small trial from the group that developed the drug.[4]
  • Different molecule (epithalamin, not epitalon): a report from the same institute followed 266 elderly people for 6 to 8 years and reported 1.6 to 1.8-fold lower mortality with epithalamin and 4.1-fold lower with thymalin plus epithalamin. The abstract does not describe randomisation or blinding, and no independent group has replicated it.[11]
  • Different molecule (epithalamin, not epitalon): small studies in elderly people reported effects on night-time melatonin (healthy elderly subjects), blood pressure and lipids (women with heart disease). In a randomised treatment study of 100 women with advanced cervical cancer, epithalamin was one part of a combination regimen, so its own effect cannot be separated.[12][13][14]

Known risks and side effects

  • FDA found no clinical data to support safety in humans, especially by subcutaneous injection, and noted possible immune reactions from aggregation and impurities.[1]
  • FDA said the mouse studies were too limited (fixed dose, females only, short exposure) to inform genotoxic or cancer risk. It noted epitalon activates telomerase and lengthens telomeres, and that longer telomeres are generally associated with higher cancer risk.[1]
  • In cultured breast cancer cells, epitalon extended telomeres, so a growth-promoting effect in cancer cells has not been excluded.[15]

Legal status

  • As of 2026-10-09 epitalon is not an approved drug. FDA's category 2 page lists epitalon among nominated-but-withdrawn substances, citing immunogenicity risk and no safety information for the proposed route.[16]
  • FDA's briefing document for the July 23-24, 2026 advisory committee evaluated epitalon only for insomnia and proposed not adding it to the 503A bulks list: it found no studies in insomnia patients, no nonclinical sleep studies, and no human safety data.[1]
  • ABC News reportedly said the committee narrowly voted to recommend adding epitalon and five other peptides to the 503A list, against FDA staff advice. This is a secondary report. FDA had not posted the outcome as of 2026-10-09. The vote is advisory and non-binding.[17]
